Welcome to Global Family Medicine

Thank you for choosing our office for all your family’s medical care needs. To assist us with your care, please arrive at least 15 minutes early to complete registration forms. Also, please bring your insurance card and a photo ID. If you are bringing your child, please bring his/her immunization records as well.

Making you feel at ease while you are in our care is one of our top priorities. We have created a comfortable environment for you and our friendly, professional staff is always available to assist you with any questions/concerns you may have.

Our doctor and nurses are experienced in the latest advancements in family and functional medicine. We believe that good medical care begins with open communication. We promise to speak candidly with you about any symptoms, our diagnosis, alternative treatments that may exist, and our recommendations. In return, we encourage you to ask any questions you may have before, during and after your treatment with us.

Appointments

Appointments are an important part of your treatment, as time has been reserved specifically for you to talk with the doctor and receive treatment. Please arrive on time for your appointment. If you are more than 15 minutes late, you might be asked to reschedule your appointment.

We request a 24-hour notice if you need to cancel or reschedule your appointment or you will be charged a cancellation/rescheduling fee. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation.

Insurance

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Please inform us of any changes to your insurance coverage.

Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If claims are not paid within 60 days, you are responsible for the balance. If you have any questions, our courteous staff is always available to answer them.

Prescriptions

If you have an upcoming appointment, we ask that you bring all prescription medications with you, even if they were prescribed by another physician. Please request all refills of continuous medicines during your appointment. In the event of your prescription expiring prior to your next appointment, first contact your pharmacy. Generally, they will request a refill from your provider.

Please allow 24 to 48 hours to fulfill all refill requests.
